Claudia Wong, FNP-BC, is an AANP Board Certified Family Nurse Practitioner specializing in the care of memory loss, mild cognitive impairment, Alzheimer’s disease and other dementias. As a Brain Health Nurse Practitioner at the Pacific Brain Health Center, Claudia works closely with physicians and other multidisciplinary team members to provide a collaborative approach to managing these neurodegenerative disorders. Claudia helps patients and their families by creating mutual goals and roadmaps through clinical evaluation, as well as providing guidance with medical, behavioral and psychosocial recommendations. She also helps with research focusing on personalized interventions to optimize brain health in those with memory loss.
Prior to joining the Pacific Brain Health Center, Claudia worked as a Dementia Care Specialist with UCLA Alzheimer’s and Dementia Care Program under the UCLA Geriatric Medicine program. She has helped of countless families through the journey of dementia and served as guide and for patients with memory loss and dementia as well provided support for caregivers and families.
Claudia has also been involved in the research of the use of a measurement tool to evaluate patient and caregiver goals over time. She has presented at medical conferences and community events regarding the topic of dementia, held educational sessions for caregivers at caregiver bootcamps, as well as developed caregiver education materials.
Claudia completed her Master of Science in Nursing from the California State University of Los Angeles as a Family Nurse Practitioner in 2009. She has worked in a variety of settings such as a Cardiac Telemetry Unit as a Registered Nurse and a Nurse Practitioner in Infectious Disease, Primary Care, and for a Home Care Program where she medically managed frail, geriatric, home-bound patients who were at high risk for hospitalization.
Claudia is also a meditation and yoga teacher who integrates a holistic view of the individual and their families.
